Multiple options from one form

Hi all--

First, a working copy of the page in question, if you need it:

Now the problem:

Clients fill out the questionnaire to receive a report.  They can pay for
the report immediately by credit card, or phone/fax/mail in their payment.
So I need to allow for either possibility.

The way I'm thinking this would work would be two links at the bottom of the
questionnaire (call that page1,) saying essentially "pay now" or "pay
later."  The "pay now" link would go to the secure credit card page (page2a)
and submit the form along with the credit card info.  "The "pay later" would
go to a page listing the phone/mail/fax option (page2b), and submit the form
from a button at the bottom of the page.

What I don't know how to do is get the Submit function on pages 2a and 2b to
also submit the info from page1.  Or am I even approaching this the right
way?
