Thanks much Jay, Collette, and Frank for your input. I think I'm going to
take the cheap and easy way out and have users submit the questionnaire
first, then redirect to a payment options page where they can either submit
the payment from the secure credit card page or choose a phone or snail-mail
option. If/when the volume becomes so big that it's hard to keep track of
who's paid and who hasn't, I'll look again at the other suggestions.
