In HTML 4, you should be able to mark-up that section using the following
code:
The doctor said to me, <q> Take two of these and call me in the morning,</q>
so I did.
You would then be able to define styles for <q> in your CSS.
The <q> tag SHOULD also add appropriate quotation marks, according to the
language.
(In answer to your question, all browsers understand <span>, and if they
don't recognise the CSS, they will simply ignore your styling.

ps. There is software, although I can't remember the name, that supposedly
allows you to see your site in different browsers. It's wildly inaccurate,
though.
