Multiple forms on a page

Hello all,
Please check out this page: http://www.flextubing.com/TechRef1.asp
Check the FIRST checkbox ("Chemical Usage Guide") and click the Display
button. So far so good. Now, if you pick a chemical from the drop-down
list and click the "Usage Info" button, you LOSE the info in the check
boxes, since the 2 forms are distinct.
I need to keep the info in the check boxes (the first form), and pick up any
changes the
user's made, either checking or un-checking. How might I do this? (The page
is done is ASP, but if someone has a solution using some other technology, I
can figure out how to translate it to ASP.)
