It's a simple script in Perl or PHP - less than 20 lines.
What are the form variable names?

Thanks for the help... but there is not going to be a mailing. This is
simply a form which will direct the user to the correct page on the site.
It's necessary to do this in form as opposed to 2 separate buttons. I
realize that might be an easy way out but not in this particular case.
