Right off the top, Michael, I'd say both are true... That is, in order to
keep individual pages in a site easily accessible to bookmark surfers,
search engines, etc., and at the same time make the backend easy to maintain
and update, you've gotta invent a folder/page naming/filing system that
stays pretty much static.

At the same time, as new purposes, creativity and technology evolve, it is
predictable that this original system will become obsolete. Considering
this, it would seem the most practical approach would be to plan for the
implementation of some type of optimum cycle -- with (hopefully) long static
periods punctuated by short  periods of intense revision work.

For the record, we are facing a similar situation with a new client. Their
site was built several years ago by another WPP and last December we were
awarded the client's "makeover" contract. We have been maintaining the site
pretty much as it came to us since then while at the same time working with
the client to discover their expectations and desires.

Also at the same time, we have been "dealing" with what seems to us like a
tremendously disorganized backend of oddly named folders and 'kruft' as you
so aptly put it... wondering how to organize it more to our standards --
especially when we implement the massive design and content revisions --
without upsetting the apple cart already described.

The plan of action proposed is the solution we intend to implement. And I'd
love to hear what others have to say on this subject as well...
