Try Email.cgi version 5.0, "One of the older, if not the oldest, continually
supported CGI scripts for the Macintosh. Still free."

There's also a tutorial on writing CGI for AppleScript at

If these don't work, you might want to look into a remotely-hosted form
script; there's a big ol' list at
