Simple form question

Can someone point me in the direction of a tutorial that explains (more specifically than the general script writing pages) the
following:

I want to have a form with several text inputs. When submitted, I want a script to take the form information and put it into a
simple table structure that is printer-friendly. I am going to do this in PHP3. (Off topic, what is the lists' opinion of
Zend/PHP4?)
