Great suggestions.  Contacting the ISP has worked for me in the past.  They
cannot knowingly allow someone to use their servers for this purpose.

Grrr, how very annoying for you! Have you contacted their ISP? This is the 
traditional advice in these circumstances - I guess any self-respecting 
company isn't going to want to be potentially drawn into a legal wrangle 
between you and one of their customers. And if they won't take it down, 
their involvement might encourage the thief to do so.
