Popup menus?

Hello all. I have a question about making a popup form in html and using it 
with a submit button. What i need to know is how I can make a popup form that 
when the item is selected by clicking on a submit button an action happens 
specific to that selected item..(ex. I have a popup form with a list of 
different websites. I want to make it so that when someone selects a website 
from the popup menu and click submit it brings them to that webpage.) I have 
found many codes for this in Java but I need to be able to do this in html.. 
Anyone know how? Thanks.
