The forms capability of PDF is called FDF (though it may have changed
names). But you'd need someone capable of writing PHP or Perl pages to
accept and parse the code. It's not hard code, but it's tedious.
Why not just create the form in text format? Albeit it's ugly, you'll
be sure the underlines lined up. Then enclose it with <PRE>...</PRE>
tags. (You could simply create a link to a text file; but some users
forget a little thing called the back button on the browser. It
depends on the users in question.)
