It depends on the form you're using. If you have cgi access, then I prefer 
the all-in-one form processor from http://www.bignosebird.com/.

It will do pretty much whatever you tell it to do through some 
easy-to-configure hidden fields and also write the results to a 
tab-delimited file. I found it to be the easiest form script for 
customizing and configuring.

But, with all things web -- there are plenty of ways to do what you're 
requesting, so if this doesn't work, then you may want to send more details 
and even a url to the form for folks to look at and then offer suggestions.
