Try http://www.bignosebird.com/carchive/survey.shtml -- their scripts are 
generally some of these easiest to configure. It is cgi, so you need access 
to your cgi-bin. It's really set up for far more than just one question, 
but you never know, you may need to recycle it down the line and you can do 
that with very little effort.
