As far as I know, YOU turn those on or off in your browser.

In MSIE4 (should be similar for other versions) go to File, then Page 
Setup. Mine has:
Header:
&w&bPage &p of &P
Footer:
&b&d

I can't remember exactly what is what, but you can play with yours until 
you find out. Or remove all of them, but note what was there in case you 
want to put it back.

In Netscape 4.74, go to File, Page Setup, and they have checkboxes for what 
items you want to print or not. Can easily turn them back on later.
