Two things that jump out of the HTML: 

First, your body tag is:

<body LINK="#31007B" ALINK="#551a8b" VLINK="#D87937" BGCOLOR="#ffffff"
LANGUAGE=javascript onload="return window_onload()">

LANGUAGE=javascript doesn't belong in there. Second, there is no other
javascript in the document, so there is no return_window_onload() function
to call.

You also call an onLoad event in the FORM tag. I'm not a Javascript expert,
but I don't think that belongs in there either. Besides, as before, there is
no initDate() function elsewhere in the doc.
