i recently had that same problem...  what's happening is that the
formmail.pl script will pretty much only work on a unix server, although
there is a program for sale (there's a link to it on matt's script archive)
that will make this program work. 

there is a free program out at http://www.analogx.com that will work on nt
servers.  it's a little clunky, but it works.  i'm sure others on this list
have other ideas for you, and you can always go searching for free nt mailer
scripts, if you decide you don't like that one.  if you only have one form,
i would recommend using it.  if you've got tons of forms, it probably won't
be the best option for you.

I inherited a web site for a new client. The site has been
up for some time, previously designed by another person.
There was a simple form on it usinf Formmail.pl. Originally,
the site was on a Unix server but the host was bought out
and then switched to an NT server.

Evidentally, from that point on the form did not function
and the client did not receive their email msgs thru CGI. I
have gone thru all sorts of troubleshooting and have
determined that the script is running and that the email
address is functional. For some reasonthe info is not being
sent. Might this be a problame with the server or some other reason.
