Could you send out random cookies to each user that fills out the survey
and if that user enters again and attempts to submit another survey, the
program would detect its cookie and a message would appear that tells the
user on that particular machine they don't have access?

Just a thought - not sure if it would accomplish what you want.

I have an online anonymous survey for employees of the company that
measures their happiness, and I need a way to keep people from stuffing
the ballot box, so to speak.  It does not have to be foolproof, only
deter individuals from taking a vendetta against the management.  I was
going to use the ip's of the submissions, but the higher-ups don't seem
to like that, any other ideas?
