Usability (Testing)

I'm curious to know how many of you take active steps towards improving
the usability of the sites you produce, specifically though observing
users, do you suggest to clients that they should pay you to carry out
usability tests before going live? Or do you just make the site live
once yourself and the client is happy with it? Or does it depend on
certain factors, such as yes for big sites, no for small ones?
