page refresh after form

I'd like to include a single value form for 
email opt-in newsletter. 

I've seen it before where once the visitor enters
their email for "opt-in" and then hits submit . . .

The page seems to refresh and now where the form
used to be is a nice 2 lined blirb thanking them for
signing up for the newsletter.

How is this accomplished?
