CGI Script Testing Request

First off, I'd like to thank everyone who helped me by testing my cgi-script
mail a file form last week.  I believe I have fixed this but I need your
help to be sure since I could not replicate the problem on my machines
before.  If you have a moment, especially those of you who tested the form
before and received error messages, please test one more time at
