David,

Sounds to me like you may have had an update in the perl version on your
server.  You may want to look into that.  The other option is that the newer
versions of IE don't pass some required variable.  I see the same error
message that you mention, even with my Firewall turned off, so it's not
related to a firewall blocking information (at least I don't think so).

You may want to consider using an alternative means for the form's
programming.  Do you have php available on your web server (if it's UNIX,
you probably do).  If so, the form is much easier to manipulate and
everything (the programming and page itself), can all be contained in one
file.
