   Once the form mail script is on the server you only need to call it from 
the cgi / bin .. actually you refer to it with a web addy like any "link" 
the form itself is written in HTML ... this site is the best I have found on

forms are not a big production once you have a cgi/bin on your server and 
they install the scripts for you ... all you do is create the form for the 
page using the tags found in that tutorial ... same ones you use for a 
mailto kind of form ... you have made it far more complicated than it need 
be ... the server admin has done the work .. all you need to know is they 
have it there , the permissions set and the path to it on the server ...
