Form problem

I've got a problem that I can't figure out -- I need somebody else's eyes 
or maybe a brain less fried than mine.

I have a form at http://www.bkrweb.com/progress/cits/contact.html .  This 
is a temporary location.  My problem is with validation.  When I run this 
through the w3c validator I get a number of messages like this: ... 
alue="http://www.cits-inc.com/images/dishback.gif">   The little arrow 
underneath is pointing at the closing bracket in each case.  Could someone 
look at my code and see if you can tell what it is I'm doing wrong?  Thanks.
