There are extensive articles on my site dealing with the myriad legal and
business issues involved in the complex area of web design relationships.
And if you have been on this list for any period, you will know how
strongly I am opposed to non lawyers using "forms" that have come from
other non lawyers.  Anyone that has paid an Internet lawyer to create an
agreement is surely not going to be willing to give you that agreement for
free.

Then if this is so, you should seek professional, experienced assistance to
help you out.  Do yourself a favor and do it right.
