Margie,
I have done a lot with Adobe PDF file on the web and I find that the =
best
and easiest way (K.I.S.S. method employed) is to:
1. Add the email address that the form (prior to PDF conversion) needs =
to be
submitted to at the bottom of the form OR just include a link off the
webpage that it is obtain at.
2. The most important and customer friendly aspect of this process is =
NOT
just create a PDF from a scanned document but to turn that simple PDF
document into an editable form. In order to accomplish this you will =
need a
full version of Adobe Acrobat.=20
Yes most companies just put the form out there for people to download,
printout and mail or send in. The most customer friendly way (I MY =
OPINION)
is to create the editable form, in this case the application; that way =
once
the form is filled out all the person needs to do to submit it is save =
the
file on their local PC and then email it back to the company or who =
ever...
I hope that this helps... If you have any more questions or want to see =
a
sample just let me know.

Subject: Adobe Acrobat: Online Completion Forms
Hello All,
A client of mine represents a personnel firm and she
would like to have the application forms posted on her
site in such a way that the applicant can complete the
pdf form online, then submit it by clicking a submit
button. These pdf files are just fine now, but need
to be printed out then completed offline and faxed or
snailmailed in.
I have done a bit of php after creating an online
form, which parses and submits to an email address.=20
However, she would like to retain the pdf format. I
haven't done this before and am wondering the best way
to go about it. Do any of you have suggestions?
Thanks in advance,
Margie
