Jimmy,
If you have your form laid out with a complex table (e.g. using several
rowspans and colspans) this can often prevent the form elements appearing in
NS4.n, though they are there and can be clicked on. Try simplifying or
removing the table and see if they reappear. And of course, validate your
page as other have suggested - any mistakes in the table coding and NS will
be the first to let you know (which is a Good Thing.).
