I've used pair.com for all of my hosting needs, and several clients' hosting
needs, for almost five years now.  They include MySQL as part of their
Webmaster account.  I've always paid them via credit card, so I'm not sure
if they take paper checks or electronic checks ... you can ask their billing
department, though.
