Extending forms

I have been using Matt's formmail script but have come up against this dilemma.
I am using the form for users to submit service call requests. The form
then gets emailed to Person#1 who forwards it to Person#2 who then forwards
it to one of the techs. What I would like is additional field titles to be
automatically printed in the text of the email so the appropriate person
can add his/her bit to the email. Is there a way to add them to the web
page so that they are included in the email but not editable by the requestor?
Below is the form result that is emailed to Person #1 (between the dashed
lines are the fields submitted by the form).
The lines below the dash are the ones I would like to have added so that
staff can input their comments.
