Generally I write such things myself, using cookies.  I also have
UBB.Threads running, which does the trick too for registered users, and
their voting behaviour gets stored in the database.

However, just remember there are ways around: clearing cookies, registering
multiple usernames, using different computers,...

I am finding I am in need of a simple, 5-10 question poll for a site. I 
could accomplish this with a form, but need to limit it to one vote per 
person.

Anyone have a good no or low-cost solution for this?
