Hello all.

I'm working on a software survey and need to offer a text field for the
user to add an entry. And I haven't done much with forms in the past.

I'm interfacing with PHP and postgres. I need the text field to _only_
contain a value if it's been changed. And if the radio button next to it 
is clicked. (This is to eliminate multiple votes per session).

Anyone have any suggestions? I'm only passing two variables so having the
radio button pass it's name and the text field passing it's name and value
wouldn't work.
